Purpose of the role:

  • To be the primary service provider for clients. Taking a holistic view of service delivery in assisting them to live independently.
  • To provide a client/family/whanau centred, safe, effective outcome focussed support/service where the majority of time is client related
  • To work with the client / their support network to identify needs and provide fundamental service and support to achieve best outcome towards a life without limits.
  • Empower the client and assist in the development of a plan around individual identified needs and priorities and ensure the effective delivery of appropriate services in line with the client plan.
  • Facilitate coordination of the services of specialised providers.
     

What we are looking for:

We are currently recruiting a part-time Primary Service Provider to join our team in Wellington. 

Primary Service Providers work directly with our clients who are blind or have low vision. This role focuses on being the primary service provider for clients to identify needs and provide fundamental service and support to achieve best outcome towards a life without limits. You will be part of both the local multi-disciplinary team and a national team of Primary Service Providers. 

To be the successful candidate we are looking for:

  • Proven experience in service delivery within the disability field, general health or rehabilitation, or a relevant tertiary education. 
  • In-depth understanding of the transdisciplinary services provided by the Blind Foundation – Training will be provided if required.
  • Someone who is able to work effectively with people from a variety of backgrounds and cultures.
  • You will also be an enthusiastic, honest and open communicator, with proven skills in client data base use and Microsoft Office.

This position covers the wider Wellington region. The ability to help out in other regions, work outside normal office hours and occasionally stay away overnight, is essential.
